ABSTRACT

Abstract Introduction Primary ciliary dyskinesia (PCD) is a rare inherited disease associated with impairment of mucociliary transport and, consequently, with a high incidence of chronic rhinosinusitis. For patients with chronic rhinosinusitis who remain symptomatic despite medical treatment, endoscopic sinus surgery is a safe and effective therapeutic option. However, to date, no studies have been found evaluating the effect of surgery on the quality of life associated with the effect on olfaction and nasal endoscopy findings of patients with primary ciliary dyskinesia and chronic rhinosinusitis. Objective To describe the effect of endoscopic sinus surgery on the quality of life, on olfaction, and on nasal endoscopy findings of adults with PCD and chronic rhinosinusitis. Methods Four patients who underwent endoscopic sinus surgery were included. The Sinonasal Outcome Test-22 (SNOT-22) score, the Nasal Obstruction Symptom Evaluation (NOSE) questionnaire, and the Lund-Kennedy score were collected preoperatively and at 3 and 6 months postoperatively. The olfaction as assessed with the University of Pennsylvania Smell Identification Test (UPSIT), which was administered preoperatively and 3 months postoperatively. Results A total of 4 patients with a mean age of 39.3 years old (3 men and 1 woman) completed the study. All patients showed clinically significant improvement in the SNOT-22, NOSE, and Lund-Kennedy scores at 3 months postoperatively, and this improvement was sustained throughout the follow-up period. However, olfaction did not improve after surgery. Conclusion The endoscopic sinus surgery treatment of chronic rhinosinusitis in adults with PCD was associated with improvement in quality of life and endoscopic findings. However, no improvement in olfaction was demonstrated. Studies with a larger number of patients and control groups should help confirm these findings.

Introducción: las variantes anatómicas nasosinusales pueden ser una causa frecuente de infecciones crónicas, y resulta importante identificarlas en la práctica diaria. Objetivo: determinar la asociación entre las variantes anatómicas del complejo osteomeatal (COM) y el desarrollo de patologías inflamatorias nasosinusales. Materiales y métodos: estudio de casos y controles, muestra de 226 pacientes identificando las variantes anatómicas del COM en la tomografía computada (TAC) de senos paranasales (SPN) y su correlación clínica. Resultados: el 51,9 % presentaron hallazgos imagenológicos indicativos de patología inflamatoria nasosinusal y el 19,8 % reportaron sintomatología sugestiva de sinusitis en la historia clínica. Los SPN más afectados fueron: maxilares (46,9 %) y etmoidales (23 %). Las variantes anatómicas más frecuentes fueron las celdillas de Agger Nasi (50,2 %) y la desviación septal (46,2 %). Se encontró como variable estadísticamente significativa la inserción lateral de la apófisis unciforme (p = 0,015) más frecuente del lado izquierdo (p = 0.018, odds ratio [OR] = 4,078, intervalo de confianza [IC] 95 % = 1,3-12,6). Discusión: Se confirmó la incidencia de las variantes anatómicas más frecuentes en la literatura, sin embargo, no se correlacionan con los hallazgos clínicos para la serie de pacientes estudiada en comparación con otros estudios. Existe una alta relación entre la inserción lateral de apófisis unciforme y hallazgos de rinosinusitis escasamente documentados en la literatura médica. Conclusión: se requieren más estudios sobre modelos predictivos en muestras poblacionales mayores y protocolos de lectura TAC enfocados sobre diferentes variantes anatómicas de la apófisis unciforme.


Introduction: Sinonasal anatomical variants can be a frequent cause of chronic in- fections, so it is important to identify them in daily practice. Objective: To determine the association between the anatomical variants of the osteomeatal complex (OCM) and the development of sinonasal inflammatory pathologies. Materials and methods: Case-control study, a sample of 226 patients is analyzed identifying the anatomical variants of OCM in computed tomography of the paranasal sinuses and their clinical correlation. Results: 51.9% presented imaging findings indicative of sinonasal in- flammatory disease, 19.8% reported symptoms suggestive of sinusitis in the clinical history. The most affected paranasal sinuses were: maxillary (46.9%) and ethmoid (23%). The most frequent anatomical variants were Agger Nasi cells (50.2%) and septal deviation (46.2%). The lateral insertion of the uncinate process (p=0.015) was a statistically significant variable, more frequent on the left side (p=0.018, odds ratio [OR]=4.078, 95% confidence interval [CI]=1.3-12.6). Discussion: The incidence of the most frequent anatomical variants in the literature was confirmed, however not correlated with the clinical findings for the series of patients studied in comparison with other studies. There is a high relationship between the lateral insertion of the uncinate process and rhinosinusitis findings that are scarcely documented in the medical literature. Conclusion: More studies are required on predictive models in larger population samples and tomographic reading protocols focused on different anatomical variants of the uncinate process

Myxomas are benign and locally invasive neoplasms of primitive mesenchymal origin. They are most commonly found in the atria of the heart. They have also been reported in bones, muscles, and other connective tissues. Myxomas are rarely found in the nose and paranasal sinuses. Myxomas are notorious for their recurrence after excision and local invasion. We report a case of intranasal myxoma in a 30-year-old male who presented with blocked ear sensation and progressive nasal block. Introducción: La cirugía endoscópica nasosinusal ha evolucionado de forma importante desde su creación. La posición anatómica que ocupan las cavidades nasosinusales con importantes estructuras adyacentes hacen que sus complicaciones, aunque raras, traigan secuelas irreversibles e incluso la muerte del paciente. Objetivo: Realizar una revisión sobre los tipos de complicaciones, frecuencia y su manejo durante la cirugía endoscópica nasosinusal. Métodos: Revisión documental en bases de datos bibliográficos sobre el tema durante el período de diciembre 2021 a enero 2022. Se seleccionaron 27 artículos relacionados con el objetivo propuesto y se desarrolló un documento resumen con la información recolectada. Desarrollo: La clasificación más utilizada separa las complicaciones en tres grados de severidad y las tasas de complicaciones mayores están por debajo del 1 por ciento en manos de cirujanos experimentados. Reconocer los sitios de mayor riesgo y los síntomas que puedan aparecer en el paciente son fundamentales para el rápido manejo de la complicación. Conclusiones: La cirugía endoscópica nasosinusal ha demostrado ser una excelente herramienta para el tratamiento quirúrgico de las enfermedades de estas cavidades en manos experimentadas. Aunque las tasas de complicaciones son ínfimas, saber reconocerlas y tratarlas a tiempo resulta fundamental(AU)


Introduction: Endoscopic nasosinusal surgery has evolved significantly since its inception. The anatomical position of the nasosinusal cavities, with important adjacent structures, means that its complications, although rare, bring about irreversible sequelae, and even death, for the patient. Objective: To review the types and frequency of complications, as well as their management, during endoscopic nasosinusal surgery. Methods: A documentary review on the subject was conducted in bibliographic databases during the period from December 2021 to January 2022. Twenty-seven articles related to the proposed objective were selected, while a summary document was developed with the collected information. Development: The most commonly used classification separates complications into three degrees of severity and includes major complication rates below 1 percent in the hands of experienced surgeons. Recognizing the sites of greatest risk and the symptoms that may appear in the patient are fundamental for the rapid management of the complication. Conclusions: Endoscopic nasosinusal surgery has proven to be an excellent tool for the surgical treatment of the diseases of these cavities in experienced hands. Although complication rates are minimal, it is essential to know how to identify and treat them(AU)

Abstract Introduction Primary ciliary dyskinesia is a rare inherited disease that results in a malfunction of mucociliary clearance and sinonasal complaints. Aplasia/hypoplasia of the frontal and sphenoid sinuses has been described as more frequent in this population. However, to date, no studies have provided a detailed description of computed tomography findings in adult patients with a diagnosis of this condition. Objective To describe the computed tomography (CT) findings of adult patients with primary ciliary dyskinesia. Methods Retrospective observational study of adult patients with primary ciliary dyskinesia who underwent CT. Results Twenty-one adults were included in the study. Aplasia occurred in 38.1% of frontal sinuses and in 14.3% of sphenoid sinuses. Likewise, hypoplasia occurred in 47.6% of the frontal sinuses, in 54.8% of the sphenoid sinuses and in 40.5% of the maxillary sinuses. Furthermore, trabecular loss was identified in 61.9% ethmoidal sinuses. The mean Lund-Mackay score was 13.5. In addition, 9.5% of the patients had concha bullosa, 47.6% had marked bilateral inferior turbinate hypertrophy, 38.1% had marked middle turbinate hypertrophy, and 47.6% had marked septal deviation. Finally, we identified images suggestive of fungus ball, mucocele, osteoma, a possible antrochoanal polyp, and frontal bone erosions. Conclusion The present study provides a detailed description of CT findings in patients with primary ciliary dyskinesia. We also describe abnormalities that must be identified for safer surgical planning and that suggest a diagnosis of primary ciliary dyskinesia if found in patients with a consistent clinical picture.

ABSTRACT Glomangiopericytoma is a rare vascular neoplasm of the nasal cavity and paranasal sinuses that occurs during the sixth or seventh decade of life. It is categorized as a borderline tumor with low malignant potential and classified as a distinct entity of sinonasal tumors with perivascular myoid phenotype by the World Health Organization (WHO). We report the case of a 50-year-old woman with nasal obstruction and severe epistaxis. The nasal sinuses computed tomography (CT), and magnetic resonance imaging (MRI) demonstrated a 3.1 cm soft tissue mass occupying the upper part of the left nasal cavity invading the left paranasal sinuses and nasal septum, and the left eye medial rectus muscle. A total mass resection was performed by nasal endoscopy. The histological and immunohistochemical examination yielded the diagnosis of glomangiopericytoma. This case report aims to contribute to the knowledge of nasal neoplasms. The need for more data on this entity is the main obstacle to developing standardized treatment guidelines.

Introducción: el tumor primario de células gigantes de tejido blando de bajo potencial maligno es un tumor raro. Se han reportado en varios sitios, incluyendo mama, glándulas salivales, pulmón, entre otros. En el cráneo representan el 1 % y afectan preferentemente al esfenoides y los huesos temporales con bajo potencial de transformación maligna. Caso: se presenta el caso de un paciente masculino de 27 años con disminución de agudeza visual izquierda rápidamente progresiva, con evidencia de defecto pupilar aferente izquierdo. La tomografía computarizada (TC) y resonancia magnética nuclear (RMN) muestran una lesión tumoral en topografía esfenoidal izquierda con extensión hacia el seno cavernoso del mismo lado que desplaza la hipófisis. Discusión: el objetivo es describir la frecuencia de la enfermedad y las características en su presentación, definir pautas para el abordaje, tratamiento y seguimiento; asimismo, establecer los factores pronósticos. Conclusiones: tumor de ubicación y presentación inusual.


Introduction: Primary tumor of giant soft tissue cells with low malignant potential is a rare tumor. They have been reported in several sites, including breast, salivary glands, lung, etc. Giant cell tumors of the skull represent 1%, preferably affects the sphenoid and temporal bones. With low potential for malignant transformation. Methods: The case of a 27-year-old male patient is presented, with decrease in left visual acuity, rapidly progressive; with evidence of left afferent pupillary defect. CT and NMR are requested finding tumor lesion in left sphenoid topography with extension to the cavernous sinus of the same side displacing the pituitary gland. Discussion: The objective of the case report is to describe the frequency of the di-sease and the characteristics in its presentation, define guidelines for the approach, treatment and follow-up; also establish the prognostic factors. Conclusions: Tumor of unusual location and presentation.

Abstract Introduction The maxillary sinus and its variations are very important to dentistry and rhinology. Objective To investigate the effect of the accessory maxillary ostium (AMO) on the variations of adjacent structures of the maxillary sinus. Methods The computed tomography (CT) images of 400 patients were retrospectively evaluated. The prevalence of AMO was calculated. The relationship between morphological variations of adjacent structures of maxillary sinus such as agger nasi cell (ANC), Haller cell (HC), nasal septum deviation (NSD), hypertrophy of inferior concha (HIC), pneumatization of middle concha (PMC), mucus retention cyst (MRC), mucosal thickening (MT), and maxillary sinusitis (MS), as well as the presence of AMO, were investigated. Results Presence of AMO was diagnosed in 42 patients (10.5%), having been found in 4.5% of the patients only on the right side, in 1.25% of the patients only on the left side, and in 4.75% of the patients on both sides. There is an increasing incidence of ANC, HC, NSD, HIC, and PMC in the presence of AMO and MS. There is a decreasing incidence of MRC in the presence of AMO. Furthermore, AMO does not affect the incidence of MT. Conclusion This study showed that most parameters, except for MRC and MT, had increasing incidence in the presence of AMO. It is important for radiologists and rhinologists to have knowledge about the location of AMO and the presence of variations of MS adjacent structures to avoid surgical complications.

Abstract Introduction Anatomical variations of the nasal cavity and of the paranasal sinuses are frequently encountered and play an important role in dysfunctional drainage of sinuses. However, it is not clear in the literature whether they predispose to sinus pathology. Objectives The aim of the present review is to summarize the understanding of the association between anatomical variations of the sinonasal area and sinus pathology. Data Synthesis The present review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. We performedathorough research on PubMed from October2004 until May 2020 byusing the search terms paranasal sinus anatomical variations and sinus disease, sinusitis, and mucosal disease. Thirty studies were eligible and were included in the analysis. Overall, the studies encompassed a total of 6,999 patients included in the present review. In many studies, it has been statistically established that certain anatomical variations increase the risk of sinus disease. On the other hand, the rest of the collected studies failed to show any statistically significant correlation between anatomical variants and sinus pathology. Conclusion The present study highlights the possible correlation between some anatomical variations of the sinonasal area and pathologies of the paranasal sinuses. Careful assessment and computed tomography (CT) in patients with chronic rhinosinusitis is needed, especially in those undergoing endoscopic surgery, to identify and treat anatomical variations in the paranasal sinuses that may be correlated with rhinosinusitis. Due to contradictory results in the literature, further research is needed to elucidate the effects of anatomical variants of the sinonasal area.

El sarcoma de ewing es un tumor maligno de rápido crecimiento, con prevalencia de 1-5 casos por cada 1.000.000 habitantes, su forma extraesquelética en la cavidad sinonasal o senos paranasales es inusual. Objetivo: describir la localización atípica de esta neoplasia y la importancia de lograr un diagnóstico oportuno. Paciente femenina, con una masa en la cavidad nasal derecha de dos meses de evolución, cefalea y epistaxis. Con asimetría en región orbitaria derecha y deformidad del tabique nasal, senos paranasales con sintomas de obstrucción. La tomografía reveló una masa que invade senos paranasales. La biopsia mostró un sarcoma de Ewing. Se confirmó con CD99. La paciente recibió quimioterapia y plan de resección quirúrgica, pero falleció. El diagnóstico y tratamiento oportuno del sarcoma de ewing en cavidad sinonasal debe apoyarse con examenes tomográficos, histopatológicos, inmunohistoquímicos y de ser posible citogenéticos para llegar al diagnóstico definitivo en etapas tempranas del tumor


Ewing's sarcoma is a rapidly growing malignant tumor, with a prevalence of 1-5 cases per 1,000,000 inhabitants, its extraskeletal shape in the sinonasal cavity or paranasal sinuses is unusual. Objective: to describe the atypical location of this neoplasm and the importance of achieving a timely diagnosis. Female patient, with a mass in the right nasal cavity of two months of evolution, headache and epistaxis. With asymmetry in the right orbital region and deformity of the nasal septum, paranasal sinuses with symptoms of obstruction. Tomography revealed a mass that invades the paranasal sinuses. The biopsy showed Ewing's sarcoma. It was confirmed with CD99. The patient received chemotherapy and a surgical resection plan, however she died. The timely diagnosis and treatment of Ewing's sarcoma in the sinonasal cavity should not be based solely on clinical evaluation, it requires a tomographic, histopathological, immunohistochemical and, if possible, cytogenetic examination to reach a definitive diagnosis in the early stages of the tumor.

ABSTRACT

Se presenta el caso de un paciente varón de 18 años, con un gran mucocele frontoetmoidal derecho, postoperado en dos oportunidades anteriores, que acudió a nuestro servicio por un empeoramiento de la diplopía. Al examen físico se visualizaba un desplazamiento del globo ocular hacia abajo y afuera. Se le realizó estudios de imágenes, una tomografía computarizada y una resonancia magnética nuclear que sugerían un mucocele frontoetmoidal derecho. Se le realizó una sinusotomía tipo Draf III para drenaje de la lesión, con mejoría de los síntomas.


We present the case of an 18-year-old male patient with a large right frontoethmoidal mucocele, postoperatively on two previous occasions, who came to our department due to worsening diplopia. Physical examination revealed a downward and outward displacement of the eyeball. Imaging studies, computed tomography, and magnetic resonance imaging were performed that suggested a right frontoethmoidal mucocele. A type Draf III sinusotomy was performed to drain the lesion, with improvement of the symptoms.

Abstract Background Multiple sclerosis (MS) is usually described as an autoimmune disease, although the exact mechanism of the disease remains unknown. There have been studies reporting that venous flow abnormalities may be involved in the pathogenesis of MS or many of the associated clinical manifestations. Objective The aim of this study was to evaluate flow volumes of the middle cerebral artery (MCA), transverse sinus (TS), and cerebral aqueduct using phase contrast magnetic resonance imaging (PC-MRI) in relapsing-remitting MS patients and a control group. Methods We included 34 patients diagnosed by the McDonald criteria, revised in 2017, as well as 15 healthy controls matched by age and sex. The MRI scans were performed using a 1.5-T superconducting scanner. Axial T1-weighted, T2-weighted, and PC-MRI sequences were performed for the quantitative investigation of flow volume measurements. Quantitative analyses of flows were performed using flow analyses program PC-MRI angiography software. A circular region of interest was placed manually into the cerebral aqueduct, bilateral MCA, and TS. Results Flow volumes of the cerebral aqueduct and MCA were not statistically significant between the MS and control groups. The flow volumes of the TS for the patient group were lower than those of the control group, and this difference was statistically significant. Conclusions A reduced TS flow volume in MS patients was noted in the present study when compared with the control group, suggesting a relation between venous pathologies and MS. Further studies are needed to understand whether this relation is causal or epiphenomenal.


Resumo Antecedentes A esclerose múltipla (EM) é comumente descrita como uma doença autoimune, embora seu mecanismo exato permaneça desconhecido. Há estudos que afirmam que anormalidades no fluxo venoso podem estar relacionadas à patogênese da EM ou a muitas das manifestações clínicas associadas. Objetivo O objetivo deste estudo é avaliar os volumes de fluxo da artéria cerebral média (ACM), do seio transverso (ST) e do aqueduto cerebral usando ressonância magnética com contraste de fase (PC-MRI) em com EM recorrente-remitente. Métodos Incluímos 34 pacientes diagnosticados pelos critérios de McDonald, revisados em 2017, além de 15 controles saudáveis pareados por idade e gênero. A ressonância magnética foi realizada usando um scanner supercondutor de 1,5 T. As sequências de PC-RM axiais, ponderadas em T1 e ponderadas em T2 foram realizadas para investigação quantitativa das medidas de volume de fluxo. As análises quantitativas de fluxo foram realizadas usando o software de angiografia PC-MRI do programa de análise de fluxo. Uma região circular de interesse foi localizada manualmente no aqueduto cerebral, ACM bilateral e ST. Resultados Os volumes de fluxo do aqueduto cerebral e da ACM não foram estatisticamente significantes entre o grupo de pacientes e os controles. Os volumes ST do grupo de pacientes foram menores que os do grupo controle, e essa diferença foi estatisticamente significante. Conclusões No presente estudo, um menor volume de fluxo ST foi registrado em pacientes com EM em comparação ao grupo controle, sugerindo uma relação entre patologias venosas e EM; Mais estudos são necessários para entender se essa relação é causal ou um fenômeno secundário.

Background:Head and neck lesions are commonly encountered in patients across all age groups. This region encompass a multitude of congenital, inflammatory or neoplastic lesions.To study different Head and neck lesions, find out frequency of benign and malignant head and neck lesions, histopathological appearance, compare and correlate the various histopathological Head and Neck lesions in relations to age, gender, and site of distribution at P. D. U. Medical College & Hospital, Rajkot, Gujarat, India. Material And Methods:Received specimens were fixed in 10% formalin overnight, processed, blocks were made and sectioning was done and stained with Harris Haematoxylin and Eosin stain in histopathology laboratory. Result:Total 1531 cases were studied during the periodof 1stAugust 2019 to 31stJuly 2021. Out of 1531 cases 991 were males and 550 females respectively. The most affected age group was 41-60 years. Most common anatomical site affected was Nose and Paranasal sinuses. This study include Infectious diseases as a leading cause of Head and Neck lesions because it includes period of Covid Pandemic with most common infection was Mucormyosis. Conclusion:We concluded that site specific data is helpful in evaluating common and rare lesions of head and neck and it also give idea about which site affects more frequently. We also concluded that period during which study is conducted is having huge impact on result.

Abstract This study aimed to describe the imaging aspects of a concha bullosa discovered incidentally by means of cone-beam computed tomography (CBCT) imaging. A female patient underwent a CBCT exam, and a bilateral extensive pneumatization of the middle concha was verified, presenting a larger dimension of the right side and a deviated septum. The patient reported nasal obstruction and loss of smell. The CBCT for covering the maxillofacial region allowed the discovery of this incidental finding, allowing the patient to be referred for appropriate treatment.


Resumen Este estudio tuvo como objetivo describir los aspectos imaginológicos de una concha bullosa hallada incidentalmente por medio de imágenes de tomografía computarizada de haz cónico (TCHC). Una paciente de sexo femenino fue sometida a examen de TCHC y se verificó una neumatización extensa bilateral de la concha media, presentando una mayor dimensión del lado derecho, asi como desvío del tabique. El paciente refirió obstrucción nasal y pérdida del olfato. La TCHC de la región maxilofacial permitió localizar este hallazgo incidental, permitiendo derivar al paciente para el tratamiento adecuado.

Resumen Introducción: El trasplante renal corresponde al tratamiento de elección para la enfermedad renal crónica. No existe un protocolo universalmente aceptado para la evaluación otorrinolaringológica del receptor del riñón a implantar, existiendo una gran heterogeneidad en su práctica. La tomografía computada de cavidades paranasales es uno de los estudios más habitualmente utilizados para descartar patologías otorrinolaringológicas que contraindiquen la intervención. Objetivo: Describir los hallazgos imagenológicos de las tomografías computadas solicitadas como evaluación pretrasplante renal. Determinar si estos hallazgos condicionaron una contraindicación para trasplante o algún cambio en el manejo del paciente. Material y Método: Estudio descriptivo de corte transversal mediante la revisión de fichas clínicas de pacientes derivados a estudio pretrasplante renal durante el año 2018 en el Hospital Carlos Van Buren. Resultados: Se obtuvo información de 40 pacientes derivados para evaluación otorrinolaringológica. El promedio de edad fue de 49 ± 11,4 años; 55% fueron mujeres. La causa más frecuente de enfermedad renal fue idiopática (70%). A 34 de 40 pacientes se les solicitó evaluación tomográfica. A cinco pacientes se les indicó corticoides intranasales y se derivó un paciente a evaluación dental. No se generó ninguna contraindicación para el trasplante renal. Discusión: Existe poca literatura sobre la utilidad de la tomografía de cavidades paranasales como estudio pretrasplante renal. En el presente estudio no se encontró ningún hallazgo que contraindicara la intervención. Conclusión: Se necesitan más estudios para poder asegurar si la evaluación otorrinolaringológica y el uso de tomografía tiene alguna implicancia en la evolución de los pacientes sometidos a trasplante renal.


Abstract Introduction: Kidney transplantation is the treatment of choice for chronic kidney disease. There is no universally accepted protocol for the otorhinolaryngological evaluation of the recipient, and there is heterogeneity in clinical practice. Computed tomography of the paranasal cavities is one of the most commonly used studies to rule out otorhinolaryngological pathologies that contraindicate the intervention. Aim: To describe the imaging findings of the computed tomographies requested as a pre-transplant evaluation. To determine if these findings determined a contraindication for transplantation or any change in the patient's management. Material and Method: Descriptive cross-sectional study by reviewing the clinical records of patients referred to a pre-kidney transplant study during 2018 at the Hospital Carlos Van Buren. Results: Information was obtained from 40 patients referred for otorhinolaryngological evaluation. The average age was 49 ± 11.4 years; 55% were women. The most common cause of kidney disease was idiopathic (70%). 34 of 40 patients had a computed tomography. Five patients received intranasal corticosteroids and one patient was referred for dental evaluation. There were no contraindications for renal transplantation. Conclusion: There is little literature on the usefulness of paranasal cavity tomography as a pre-kidney transplant study. In the present study, no finding was found that would contraindicate the intervention. More studies are needed to be able to ascertain whether the otorhinolaryngological evaluation and the use of tomography have any implications in the evolution of patients undergoing kidney transplantation.

ABSTRACT

Introducción: Los quistes de retención mucoso de los senos paranasales son un hallazgo imagenológico incidental muy frecuente. Usualmente comprometen el seno maxilar. Metodología: Se realizó una amplia búsqueda de la literatura, sobre quistes de retención mucoso nasosinusales, en enero de 2021 en múltiples bases de datos. Se seleccionaron 21 artículos sobre el tema para la realización de la revisión. Discusión: En la mayoría de los casos el tratamiento de los quistes de retención mucoso nasosinusales debe ser expectante debido a su tendencia a la involución y curso asintomático. Sin embargo, en la práctica clínica se observa con frecuencia un manejo quirúrgico sin una indicación clara. Conclusiones: Las indicaciones quirúrgicas de los quistes de retención mucoso son: obstrucción del ostium del seno paranasal, compromiso del complejo ostiomeatal o compromiso del nervio infraorbitario con síntomas secundarios. Es importante que el cirujano reconozca la naturaleza y comportamiento de esta patología para evitar procedimientos quirúrgicos innecesarios.


Introduction: Mucous retention cysts of the paranasal sinuses are a very common incidental imaging finding. They usually involve the maxillary sinus. Methodology: An extensive literature search on nasosinusal mucosal retention cysts was conducted in January 2021 through multiple databases. 21 articles on the subject were included as evidence for this paper. Discussion: In most cases, treatment should be expectant due to its tendency to involution and asymptomatic course. However, in clinical practice, surgical management is frequently observed without a clear indication. Conclusions: Surgical indications for mucosal retention cysts are: obstruction of the sinus ostium, involvement of the ostiomeatal complex, or involvement of the infraorbital nerve with secondary symptoms. It is important for the surgeon to recognize the nature and behavior of this pathology in order to avoid unnecessary surgical procedures.

Introducción: la revolución tecnológica que hemos vivido en los últimos años ha generado un extendido uso de la inteligencia artificial (IA) como tecnología de base para el desarrollo de diversos sistemas y soluciones en medicina. En el campo de la otorrinolaringología, estamos viendo hasta ahora los primeros esfuerzos para aprovechar este surgimiento. Objetivo: el presente proyecto busca describir el proceso de desarrollo de una app móvil creada gracias al trabajo colaborativo entre otorrinolaringólogos e ingenieros biomédicos, que tiene la intención de optimizar la evaluación preoperatoria de la tomografía de senos paranasales (TC de SPN). Métodos: el desarrollo de la app siguió el método de priorización para especificaciones de MoSCoW. Utilizamos la información recolectada de encuestas realizadas a 29 expertos en rinología de diferentes partes del mundo, quienes evaluaron variantes anatómicas en la tomografía y se utilizaron dos modelos de regresión para la predicción de dificultad y riesgo a partir de herramientas de aprendizaje estadístico. Conclusión: hemos desarrollado una herramienta y un modelo estadístico que es fácil de utilizar y que idealmente ayudará al cirujano en el proceso de evaluación preoperatoria de TC de SPN. Este es un ejercicio que permite demostrar la eficacia de un desarrollo colaborativo para lograr soluciones en nuestra especialidad y un acercamiento hacia la IA.


Introduction: The recent technology revolution that we have experienced has generated extensive interest in the use of artificial intelligence (AI) in the development of various systems and solutions in medicine. In the field of Otorhinolaryngology, we are seeing the first efforts to take advantage of this flourishing area. Objective: We sought to describe the development process of a mobile app created through a collaborative effort between ENT surgeons and biomedical engineers. This app has the intention to optimize the preoperative evaluation of paranasal sinus tomography (CT) to improve safety and outcomes in Endoscopic Sinus Surgery (ESS). Methods: The development of the app followed the prioritization method for MoSCoW specifications. We used the information collected from surveys of 29 Rhinology experts from different parts of the world, who evaluated anatomical variants on sinus CT scans. Two regression models were used to predict difficulty and risk using statistical learning. Conclusion: Via statistical modelling, we have developed a user-friendly tool that will ideally help surgeons assess the risk and difficulty of ESS based on the pre-operative CT scan of the sinuses. This is an exercise that demonstrates the efficacy of the collaborative efforts between surgeons and engineers to leverage AI tools and promote better solutions for our patients.

Dada la respiración nasal preferencial que tiene la población pediátrica en los primeros meses de vida, la obstrucción nasal tiene más repercusiones a nivel clínico a diferencia de un adulto, lo cual hace más prioritario su diagnóstico y manejo. Una de las principales causas de obstrucción nasal en niños son las masas nasales, las cuales debido a sus múltiples etiologías causan bastante incertidumbre diagnóstica en el equipo médico. En este artículo se presenta el caso de una paciente de 10 meses con una masa en el ala nasal de crecimiento rápido; se realiza un rápido diagnóstico de rabdomiosarcoma que requiere un adecuado manejo. El rabdomiosarcoma con patrón alveolar se presenta de forma inusual en esta ubicación, es por esto por lo que se considera importante presentar este caso para mantenerlo en el algoritmo diagnóstico como una posibilidad y que, así como en el caso presentado, se pueda tener un diagnóstico temprano, un tratamiento adecuado y un resultado estético y funcional favorable.


Given preferential nasal breathing in the first months of life in the pediatric population, nasal obstruction has more clinical repercussions than it would have in adults, hence the need to give higher priority to its diagnosis and management. One of the main causes of nasal obstruction in children is the presence of nasal masses, which cause considerable diagnostic uncertainty in the medical team, because of their multiple etiologies. In this article, the case of a 10-month-old patient with a rapidly growing mass in the nasal ala is presented. A rhabdomyosarcoma was promptly diagnosed and adequately managed. Rhabdomyosarcoma with an alveolar pattern occurs in an unusual way in this location; for this reason, this case is important in order to include this possibility in the diagnostic algorithm and, as was the case in this patient, reach an early diagnosis and institute adequate treatment resulting in favorable aesthetic and functional results.

Introducción: Los tumores de la cavidad nasal y senos paranasales son neoplasias poco frecuentes. La histopatología y características clínicas son diferentes al resto de las neoplasias de cabeza y cuello. Objetivo: Caracterizar desde el punto de vista clínico-epidemiológico, radiológico y anatomopatológico a los pacientes mayores de 18 años con tumores nasosinusales malignos atendidos en un hospital hondureño. Métodos: Estudio observacional, descriptivo y transversal, mediante una técnica de muestreo no probabilístico por conveniencia se tomó una muestra de 30 pacientes que estaban hospitalizados en sala de otorrinolaringología del Hospital Escuela de la Facultad de Ciencias Médicas, Universidad Nacional Autónoma de Honduras, en el periodo enero del 2017-diciembre del 2019. Los pacientes estudiados habían sido diagnosticados con cáncer de nariz y senos paranasales. Resultados: El estudio incluyó 30 pacientes, de los cuales el 47 por ciento eran mayores de 50 años, de sexo femenino un 53 por ciento, amas de casa el 40 por ciento y agricultores el 20 por ciento. Los principales síntomas y signos fueron obstrucción nasal (50 por ciento), rinorrea (30 por ciento) y masa (20 por ciento), con localización principalmente en cavidades nasales (67 por ciento). Las características radiológicas más importantes fueron su vascularidad (43 por ciento) y unilateralidad (33 por ciento), la estirpe histológica que predominó fue el linfoma (30 por ciento). Conclusiones: Los tumores nasales predominaron en el sexo femenino, siendo la obstrucción nasal, rinorrea y masa sus síntomas y signos principales. El aumento de la vascularidad en la imagen por tomografía computarizada orienta a sospechar malignidad. Se evidenció que el factor económico y el desconocimiento de la enfermedad demoró la asistencia del paciente a un centro hospitalario(AU)


Introduction: Tumors of the nasal cavity and paranasal sinuses are infrequent neoplasms. Their histopathology and clinical characteristics are different from those of other head and neck neoplasms. Objective: Carry out a clinical-epidemiological, radiological and anatomopathological characterization of patients aged over 18 years with malignant sinonasal tumors attending a Honduran hospital. Methods: A cross-sectional observational descriptive study was conducted. Nonprobability convenience sampling technique was applied to select a sample of 30 patients staying at the otolaryngology ward of the Medical Sciences Hospital School at the National Autonomous University of Honduras from January 2017 to December 2019. These patients had been diagnosed with nose and paranasal sinus cancer. Results: The study included 30 patients, of whom 47 percent were aged over 50 years, 53 percent were female, 40 percent were housewives and 20 percent were farmers. The main signs and symptoms were nasal obstruction (50 percent), rhinorrhea (30 percent) and mass (20 percent), with most common location in the nasal cavity (67 percent). The most important radiological characteristics were their vascularity (43 percent) and one-sidedness (33 percent), with lymphoma as the prevailing histological type (30 percent). Conclusions: Nasal tumors prevailed in the female sex, with nasal obstruction, rhinorrhea and mass as their main signs and symptoms. Increase in vascularity in computed tomography images leads to suspecting malignancy. Evidence was found that the economic factor and poor knowledge about the disease delayed patient attendance to a hospital institution(AU)

Resumen Los carcinomas neuroendocrinos son tumores poco frecuentes en nariz y senos paranasales. Se consideran tumores primitivos y son los menos diferenciados del sistema neuroendocrino difuso. Se discute el caso de una paciente femenina de 49 años quien acude al servicio de otorrinolaringología en el Hospital Central Militar refiriendo masa en cavidad nasal izquierda de crecimiento progresivo en un periodo de 9 meses, así como epistaxis y obstrucción nasal izquierda. Mediante resonancia magnética y tomografía computada se observó tumoración de la cavidad nasal izquierda en su totalidad hasta nasofaringe, con captación heterogénea del medio de contraste. Se decide resección de tumoración mediante abordaje endoscópico (sinusotomía maxilar tipo III, etmoidectomía anterior y posterior izquierda, Draf esfenoidal tipo II izquierdo, Draf IIa del seno frontal izquierdo, así como septectomía posterior). El estudio histopatológico reportó un carcinoma neuroendocrino moderadamente diferenciado, por lo que se trató de forma conjunta con servicio de oncología médica. Este tipo de tumores son un reto diagnostico por la complejidad para diferenciarlos en un estudio histopatológico. La diferenciación efectiva de los mismos puede tener un impacto clínico, por lo que los avances en la intervención terapéutica podrían prolongar la supervivencia del paciente, mejorar la calidad de vida e incluso la cura.


Abstract Neuroendocrine carcinomas are rare tumors of the nose and sinuses. They are considered primitive tumors and are the least differentiated from the diffuse neuroendocrine system. We report a 49-year-old female patient who attends the otolaryngology service at the Central Military Hospital referring mass in the left nasal cavity with a progressive growth over a period of 9 months, including epistaxis, as well as left nasal obstruction. Magnetic Resonance Imaging and Computed Tomography imaging studies showed a tumor occupying the entirety of the left nasal cavity to the nasopharynx, with heterogeneous uptake of the contrast medium. Tumor resection is decided by endoscopic approach (maxillary sinusotomy type III, left anterior and posterior ethmoidectomy, left sphenoid Draf type II, left frontal Draf IIa as well as posterior septectomy). The pathology service reported a moderately differentiated neuroendocrine carcinoma. The treatment was decided joint session with the medical oncology service. These types of tumors are a diagnostic challenge because of the complexity to differentiate them in a histopathological study. Their effective differentiation can have a clinical impact, that's why the advances in therapeutic intervention could prolong patient survival, improve quality of life and even find a cure.
